One of the best ways to prevent future accidents is to analyze current accidents and “near-misses” to determine what happened and how it could have been prevented. Your operations probably don’t change that quickly, so an accident today could easily happen again in the future. By finding ways to prevent the accident now, future accidents can be avoided. A proper “incident,” not just accident, investigation is necessary to accomplish this. Incidents are any occurrence that either resulted in an injury or COULD have resulted in an injury.

In order to conduct an incident investigation, you need a form that collects all pertinent information. Our Accident Investigation Form is available for you to use, but you are welcome to use your own form or any form you like as long as it collects the same information. Be sure to complete each line of the form and pay particular attention to witness statements.

We also have available safety videotapes on incident investigation that provide additional training for supervisors.
